Packer Scholarship Sign Up Form

 
All Packer Parents who received a Packer scholarship are required to sign up for EITHER shifts in the Alexander Park Concession Stand OR the Product Purchase Program.
 
Alex Park Concession Stand Shifts - Each shift is valued at $50.  EXAMPLE - If your scholarship was $100 you are required to work TWO shifts.
 
Product Purchase Program - Parents can opt to purchase drinks for the concession stand.  Each drink type has a scholarship value (Case Gatorade-$50, Case Soda-$25 3 Cases of Water-$50)  EXAMPLE - If your scholarship was $50 you can purchase 1 case of Gatorade or 2 cases of Soda or 3 cases of Water, instead of working a shift.
 
PLEASE NOTE:  If you do NOT sign up for a shift OR participate in the Product Purchase program you will be required to pay the 50% of your scholarship back to the Packers in cash, or your child will be held out of the Playoffs.
